PRIOR TO DRAFTING ANY CONTRACT FOR GOODS AND SERVICES, THE RESPONSIBLE PARTY OR DECEDENTS SURVIVOR WHO IS HANDLING THE FUNERAL ARRANGEMENTS IS ENTITLED TO RECEIVE A COPY OF ANY PRENEED AGREEMENT IN THE POSSESSION OF THE FUNERAL ESTABLISHMENT THAT HAS BEEN SIGNED AND PAID FOR, IN FULL OR IN PART, BY OR ON BEHALF OF THE DECEDENT.
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services of funeral director, staff and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
A DISCOUNT OF $600.00 WILL BE DEDUCTED FROM THE TOTAL SERVICE CHARGES
WHEN THE CASKET IS SELECTED THROUGH TODD MEMORIAL CHAPEL.
SERVICE CHARGES

BASIC SERVICES OF FUNERAL DIRECTOR, STAFF, AND OVERHEAD |

This is a fee which we charge every family for our basic services which include: consultation with the family or responsible party, clerical services, coordinating service plans with cemetery, crematory and/or other involved parties, obtaining required authorizations and permits (locally), and a fee necessary to cover a proportionate share of our overhead costs such as licenses, insurance, maintenance of facilities and equipment, utilities, and a host of other costs related to being in business to serve the public.
This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains. |

Our charge for direct cremation includes: Basic services of the funeral director. staff and overhead, local transfer to funeral home, refrigeration in lieu of embalming, and delivery van to local crematory at our convenience. These charges DO NOT include viewing, funeral ceremony, crematory fees, county or state fees, or taxes.
If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard, or composition materials (with or without an outside covering). The container we provide is cardboard. |

REGARDING WARRANTY
Todd Memorial Chapel makes no representations or warranties about the protective value of certain caskets or outer
burial containers other than those made by the manufacturer. The only warranties, express or implied, granted are
the express written warranties, if any, extended by the manufacturers thereof. No other warranties and no warranties
of merchandising fitness for a particular product are extended by Todd Memorial Chapel.
